Application for Sunny Isles Beach Resident ID Card

Required for ALL program registrations and events at the resident rate.

Please provide:

• Government issued photo identification (i.e. driver’s license, passport)
• Current Deed/Lease (duration must be 1 year). The copy of the valid lease must be accompanied by a letter from the condo/management office stating who resides in the unit.

AND

2 of the items below:

• Current Vehicle Registration AND Insurance
• Utility Bill (i.e. electric, home phone, cable) - within last 3 months
• Credit/debit card statement - within last 3 months

Person who is named on the deed/lease is considered the head of household.

Must provide birth certificate for children ages 5-17.

Adults 18 and up listed in household must provide:

• Government issued photo identification
• Marriage certificate (if applicable) or 3 different types of bills dated within the last 3 months
