Application For Community TNVR Cat Program Volunteer

Sunny Isles Beach residents must provide a copy of one of the following as proof of residency:

  • Valid City-issued Sunny Isles Beach Resident ID Card
  • Current deed or lease (duration must be at least 1 year). A copy of the lease AND a letter from the condo/management office stating who resides in the unit are required.
  • A utility bill from within the last 3 months (electric, cable, or phone) with your name and Sunny Isles Beach address
    • Note: mobile phone bills will NOT be accepted.
  • Current car registration and proof of insurance
  • A credit or debit card statement within the last 3 months

Non-residents of Sunny Isles Beach will be considered.

Volunteer Code of Conduct

  1. Volunteers assisting the City are carefully screened and undergo a thorough selection process.
  2. Volunteers shall follow guidelines, rules, and regulations established by the City of Sunny Isles Beach.
  3. Volunteers shall always conduct themselves in a professional manner.
  4. ID cards shall be worn at all times and be displayed in a visible location.
    • ID card will be revoked and deactivated if removed from the program.
    • If you voluntarily leave the program, you are required to notify SIBcatprogram@sibfl.net and return your ID card.

 

Volunteer Expectations

  1. Volunteers must apply to participate in the Community TNVR Cat Program annually.
    • ID card is only valid for the calendar year. For example, ID cards issued August 1, 2020 will expire December 31, 2020.
  2. Volunteers will be expected to report the following upon application approval and then on a monthly basis (no later than the last day of each month):
    • # of cats at their station
    • # of cats trapped/fostered/transported (Minimum of one (1) cat per month)
    • # of spays/neuters
    • # of adoptions
    • # of names and locations of new cats
  3. Any unusual activity needs to be reported immediately (e.g. sudden increase in cats, unauthorized feedings, etc.).
  4. Feeding and cleaning:
    • Feeding hours are from 6:00pm-6:00am.
    • Must stay within the boundaries of the designated and authorized feeding area.
    • Food and water must not be left out between nonfeeding hours.
    • Feeding station(s) must be cleaned daily and maintained in a clean and sanitary condition (e.g. free of cat food and water---during non-feeding hours, containers, regurgitations, and feces).
  5. Preapproval for all services including emergencies is required. Volunteers must complete the Preapproval Form and obtain a response prior to a cat receiving service. The City is not financially responsible for invoices or reimbursement requests for services that are not preapproved by a representative of the Development Services Department.
  6. Volunteers shall schedule TNVR drop offs with a City of Sunny Isles Beach representative and shall not drop off cats to contracted facilities unannounced. The Humane Society of Greater Miami will provide the approved days for drop off. Volunteer shall submit receipt for service within 24 hours of completion.
  7. Volunteers dropping off cats at contracted facilities must pick up cats on the same day unless specifically authorized by the vet. If the volunteer cannot commit to picking up the cat from contracted facilities on the same day, then they should not schedule a drop off.
  8. Volunteers are only allowed to TNVR and feed cats on designated public property. No TNVR activities or feeding on private property or non-designated public property at any time. Failure to abide will result in fines and/or criminal charges.

FAILURE TO COMPLY WILL RESULT IN REMOVAL FROM THE COMMUNITY TNVR CAT PROGRAM.

 

Indemnity Clause

I hereby certify that all information provided by me is true and correct to the best of my knowledge and any misrepresentation or omissions of fact, misleading or false information on my part will be grounds for dismissal as a volunteer. Acceptance as a volunteer is contingent upon satisfactory references, verification of the information submitted on this application and satisfactory completion of mandatory requirements, which may include drug screening and criminal background check. I authorize that all employers, schools, or references thus contacted be released from all liability in answering questions related to my application. I therefore authorize you to make such investigations and inquiries you deem necessary in arriving at a decision to accept me as a volunteer.

In consideration of the opportunity afforded me to participate as a volunteer, I do hereby knowingly, freely and voluntarily release, waive, discharge, indemnify and hold harmless the CITY OF SUNNY ISLES BEACH, its elected/appointed officials, directors, officers, employees, and agents (the “City”) from any and all liability, losses, expenses, damages, claims, causes of actions or judgments, including without limitation attorneys' fee and court costs, which may be sustained by me and /or my family directly or indirectly in connection with , or which may arise out of, my participation as a volunteer, regardless of  City’s negligence . I further agree not to present myself as an officer, agent or employee of the City and acknowledge that I am participating as a volunteer for the City on my own time and outside the scope of my employment, that I am not considered an employee of the City, and am not entitled to any City compensation, stipend or fringe benefits.

I acknowledge that I am willingly and voluntarily participating as a volunteer in the Trap, Neuter, Vaccinate, Release (TNVR) program, with knowledge of the dangers involved. I acknowledge that participation may involve risk to my personal safety and carries with it the potential for injury, death, and property loss. I understand that the City makes no guarantees that my participation is free of hazards, including by way of example and not limitation, those associated with handling animals, terrain, equipment, weather, my personal health, my physical limitations, or the actions of others, and makes no guarantee ensuring my personal safety.

I further acknowledge that as a volunteer in the Trap, Neuter, Vaccinate, Release (TNVR) program, I will be engaging in an inherently dangerous activity, because stray/feral/community cats may be aggressive, and carry diseases. I hereby agree to expressly assume and accept any and all risks of injury, illness, or death which in any way arise out of such volunteering.

I further agree to abide by the City’s safety policies and procedures, criteria and requirements in addition to all safety instructions and directions provided by City personnel during a volunteer activity.

I further agree that any vehicle that I drive to, from, or during a volunteer activity is not property of or an instrumentality of the City but rather my own property. I acknowledge that the City will not be responsible or liable for any personal injury or property damage caused in whole or in part by my personal vehicle.

I declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that would prevent from participating as a volunteer. I hereby acknowledge that it is recommended that a physician’s approval be obtained in advance of participation in the Trap, Neuter, Vaccinate, Release (TNVR) activities. I further acknowledge that I have either had a physical examination or has been given a physician’s permission to participate, or that I have decided to participate without the approval of my physician, and do hereby assume all responsibility for my volunteering in the City’s Trap, Neuter, Vaccinate, Release (TNVR) activities.


I have read this Release & Indemnification Agreement and fully understand its terms and conditions. I further understand that I have given up substantial rights by signing this form and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of, and indemnification from. Any and all liability to the greatest extent permitted by law and agree that if any portion of this Release & Indemnification Agreement is held to be invalid, the balance notwithstanding shall continue in full legal force and effect.

I HAVE READ, UNDERSTAND, AND AGREE TO THE RELEASE AND INDEMNIFICATION AGREEMENT AND TO THE VOLUNTEER PROGRAM TERMS AND CONDITIONS AND INFORMATION DELINEATED IN THIS APPLICATION.
